A utilização do patellar groove replacement (PGR) no tratamento da luxação patelar em animais de companhia

Resumo
O presente trabalho surge na sequência do estágio curricular do Mestrado Integrado em Medicina Veterinária, realizado no HVUC – Hospital Veterinário Universitário de Coimbra e na BONEMATRIX - Centro de Cirurgia Veterinária Avançada realizado entre os dias 15 de novembro de 2020 e 30 de abril de 2021 tendo como objetivo a abordagem do tema “A utilização do patellar groove replacement (PGR) no tratamento da luxação patelar em animais de companhia”. Numa primeira fase do presente trabalho foi assim realizada uma revisão bibliográficas sobre esta temática, dando especial ênfase à prótese troclear (PGR), não desconsiderando a normal anatomia da articulação do joelho, a sua biomecânica, fisiopatogenia, diagnóstico e restantes formas de tratamento. Foi também dado um especial destaque ás técnicas cirúrgicas de reconstrução óssea, nomeadamente a trocleoplastia, transposição da tuberosidade tibial e a osteotomia distal do fémur, frequentemente associadas à colocação da prótese troclear. O PGR por si só não permite a correção da luxação patelar nas situações em que esta seja provocada por graves deformações músculo-esqueléticas, estando por isso normalmente associado outras técnicas cirúrgicas aquando da sua colocação. O PGR é assim usado em situações de luxação patelar crónica com presença de osteoartrite femoro-patelar grave, onde as restantes técnicas usadas isoladamente não permitiriam a redução eficaz da patela principalmente devido as lesões provocadas na tróclea femoral. Numa segunda fase do trabalho são descritos dois casos clínicos acompanhados durante o estágio, sendo acompanhados pela sua história clínica, estudo radiográfico, diagnóstico, planeamento cirúrgico terminando com o seu tratamento.
This thesis is the result of the curricular internship of the Integrated Master's Degree in Veterinary Medicine, held at the HVUC - Hospital Veterinário Universitário de Coimbra and at BONEMATRIX - Centro de Cirurgia Veterinária Avançada fulfilled between November 15, 2020 and April 30, 2021 with the aim of addressing the topic "The use of the patellar groove replacement (PGR) in the treatment of patellar luxation in small animals". In the first phase of this work, a literature review was carried out on this subject, giving special emphasis to trochlear prosthesis (PGR), without disregarding the normal anatomy of the knee joint, its biomechanics, pathophysiology, diagnosis, and other forms of treatment. Special emphasis was also given to surgical techniques of bone reconstruction: trochleoplasty, transposition of the tibial tuberosity and distal osteotomy of the femur, often associated with the placement of trochlear prosthesis. The PGR alone does not allow for the correction of patellar luxation in situations where this is caused by serious musculoskeletal deformities and is therefore usually associated with other surgical techniques when it is placed. The PGR is therefore used in situations of chronic patellar luxation with the presence of severe patellofemoral osteoarthritis, where the other techniques used alone would not allow the effective reduction of the patella, mainly due to the lesions caused to the femoral trochlea. In a second phase of the work, two clinical cases that were followed during the internship are described, being followed by their clinical history, radiographic study, diagnosis, surgical planning ending with their treatment.
