Imagem corporal, atividade física e variáveis biossociais em diferentes momentos da menarca

Resumo
Esta tese pressupõe analisar a imagem corporal (IC), a atividade física (AF) e as variáveis biossociais em diferentes momentos da menarca (pré menarca, pós menarca e um ano após a menarca) de adolescentes de 10 a 13 anos (M =11,03; DP =0,78), com IMC de 18,98 kg/m2 (DP=3,17), das escolas da rede de ensino da cidade de Macapá/Amapá/Brasil e verificar os respectivos efeitos entre as variáveis dependentes. Para tal, foi dividida em três estudos (artigos), a saber: O Estudo (1) teve como objetivo rever a literatura sistemática e sintetizar as evidências científicas disponíveis sobre a IC e a maturação sexual de adolescentes. A discussão dos resultados, evidenciaram que os artigos que tratam da IC e maturação sexual revelaram que em função do instrumento utilizado, adolescentes apresentaram maiores níveis de insatisfação com a IC como um todo ou em partes, durante as fases púberes e pós-púberes. Nos artigos que abordaram a maturação sexual e a puberdade, os resultados evidenciaram as adolescentes com maiores níveis de insatisfação corporal, independente das variáveis que se relacionaram. Além disso, os artigos de IC e menarca confirmaram os dados anteriores, em que as adolescentes na menarca estão mais insatisfeitas com a IC. Concluiu-se com esta revisão sistemática e sintetização das evidências científicas sobre a IC e a maturação sexual, que as adolescentes independente da fase de maturação sexual estão mais insatisfeitas com a sua IC. O Estudo (2) analisou a associação entre a renda familiar (RF) e níveis de IMC com a IC e qualidade de vida (QV) em diferentes momentos da menarca; e a variação temporal da relação entre a RF e o IMC com a IC e QV em diferentes momentos da menarca. Os principais resultados foram apresentados, da seguinte forma: o IMC se relacionou positivamente com o escore total de insatisfação com a IC nos três momentos da menarca (momento 1: r=0,36; p<0,001; momento 2: r=0,44; p<0,001; momento 3: r=0,44; p<0,001) e suas dimensões específicas: auto percepção da forma/aparência corporal, percepção comparativa da IC e atitudes em relação à alteração com a IC. A análise comparativa dos escores de qualidade de vida em função dos níveis de IMC, unicamente evidenciou um efeito significativo no momento 2 (p<0,05), tendo grupo de adolescentes insatisfeitas (48,55±7,19) com sua IC revelado níveis de qualidade de vida inferiores ao grupo de adolescentes satisfeitas (53,09±7,92). A análise longitudinal dos níveis de renda familiar e IMC indicou aumento significativo nos três momentos da menarca nomeadamente: renda familiar (F=47,17; p<0,001; η2=0,26) e IMC (F=15,62; p<0,001; η2=0,11). A variação temporal da insatisfação com a IC, unicamente indicou um efeito significativo na sub escala das atitudes em relação as alterações com a IC (F=8,50; p<0,001; η2=0,06). O escore de qualidade de vida aumentou de 50,79±9,17 do momento 1 para 52,72 ±7,94 no momento 2 (p=0,037), tendo diminuído do momento 2 para 49,96±10,32 no momento 3 (p=0,007). Concluiu-se que as adolescentes revelaram uma menor insatisfação com a IC nos três momentos da menarca, evidenciada por um IMC normal; e que durante os três momentos da menarca a baixa insatisfação da IC revelou-se pela associação positiva da RF e do IMC. Já a QV identificou uma melhora entre o momento 1 e 2 da menarca, mas entre o momento 2 e 3 ocorreu uma diminuição da QV das adolescentes. O Estudo (3), analisou a influência da RF, IMC, IC e QV, mensuradas nos três diferentes momentos da menarca, na predição dos níveis de atividade física (AF) e sedentarismo um ano após a menarca. O modelo de equação estrutural mostrou-se adequado, indicando que os níveis de sedentarismo e AF mensurados um ano após a menarca foram unicamente influenciados pela RF e níveis de IMC após a ocorrência da menarca, do seguinte modo: a RF influenciou positivamente os níveis de sedentarismo e negativamente os níveis de AF moderada, enquanto o IMC influenciou positivamente os níveis de caminhada e negativamente os níveis de AF vigorosa. Assim, concluiu-se que os efeitos da RF e do IMC nos níveis de AF e sedentarismo (um ano após menarca) dependem do momento em que esta sendo avaliada, pelo que somente se verificaram influências significativas no momento logo após a menarca (momento 3). Por fim, as variáveis QV e IC mensuradas antes ou após a menarca não apresentaram qualquer tipo de efeito significativo na predição dos níveis de AF e sedentarismo das adolescentes.
This thesis aims to analyze body image (BI), physical activity (PA) and biosocial variables at different moments of the menarche (pre-menarche, post-menarche and one year after the menarche) of teenagers aged 10 to 13 years (Mean=11.03; SD=0.78), with BMI of 18.98 kg/m² (SD=3.17), from the Macapá City School District (Amapá state, Brazil), and verify the respective effects among the dependent variables. For such, the research was divided into three studies (articles), as follows: Study (1) aimed to review the systematic literature and synthetize the scientific evidence available about BI and the teenagers’ sexual maturation. The discussion of the results highlighted that articles about BI and sexual maturation, considering the utilized instruments, revealed teenagers to feature higher levels of dissatisfaction with their BI as a whole or partially, during the pubertal or post-pubertal phases. Results from articles about sexual maturation and puberty only highlighted those teenagers with higher levels of body dissatisfaction, independently from the related variables. Furthermore, the BI articles confirmed the previous data, in which the teenagers in the menarche phase were more dissatisfied with their BI. This systematic review and the scientific evidence synthesis about BI and sexual maturation concluded that teenagers are more dissatisfied with their BI, independently from the sexual maturation phase. Study (2) has analyzed the association of family income (FI) and BMI levels with BI and life quality (LQ) at different moments of the menarche; the temporal variation of the relation between FI and BMI with BI and LQ at different moments of the menarche was also analyzed. The main results were presented as follows: the BMI was positively related with the total score of dissatisfaction with the BI for all three moments of the menarche (moment 1: r=0.36; p<0.001; moment 2: r=0.44; p<0.001; moment 3: r=0.44; p<0.001) and its specific dimensions: self perception of the body shape/appearance, comparative perception of the BI and the attitudes in relation to the BI alteration. The comparative analysis of the life quality scores according to the BMI levels evidenced a significant effect uniquely at moment 2 (p<0.05), where a group of teenagers (48.55±7.19) dissatisfied with their BI revealed levels of inferior life quality in relation to the group of satisfied teenagers (53.09±7.92). The longitudinal analysis of the family income levels and the BMI significantly increased at all three moments of the menarche, as follows: family income (F=47.17; p<0.001; η2=0.26) and BMI (F=15.62; p<0.001; η2=0.11).The temporal variation of the dissatisfaction with the BI had a significant effect exclusively on the sub scale of the attitudes in relation to the BI alterations (F=8.50; p<0.001; η2=0.06). The life quality score increased from 50.79±9.17 at moment 1, to 52.72 ±7.94 at moment 2 (p=0.037), decreasing to 49.96±10.32 at moment 3 (p=0.007). In conclusion, teenagers revealed lower dissatisfaction with their BI at all three moments of the menarche, highlighted by a normal BMI; also, during all three moments of the menarche, the decrease of the BI dissatisfaction was revealed by the positive association between FI and BMI. The life quality score improved between moment 1 and 2 of the menarche; however, between moment 2 and 3, a decrease in the life quality of teenagers occurred. Study (3) has analyzed the influence of FI, BMI, BI and LQ, measured at three different moments of the menarche, in order to predict physical activity levels (PA) and sedentariness one year after the menarche. The model of the structural equation was proved suitable, signaling the sedentariness and PA levels measured one year after the menarche to be uniquely influenced by the family income and BMI levels after the menarche occurrence, as follows: FI positively influenced the levels of sedentariness and negatively influenced the levels of moderate PA, while the BMI positively influenced walking levels and negatively influenced the levels of vigorous PA. Thereby, the study concluded that the effects of FI and BMI on the levels of PA and sedentariness (one year after the menarche) depend on the moment when they are being evaluated, so that significant influences were only verified at the moment right after the menarche (moment 3). Finally, the LQ and BI variables measured before or after the menarche did not show any type of significant effect on the prediction of PA and sedentariness levels in teenagers.
