Resumo
O cancro é a segunda causa de morte na União Europeia, responsável por uma incidência de
4.000 novos casos ano e por milhão de habitantes. A radioterapia tem aplicação em 50 a 60% dos
doentes oncológicos. Com a alteração dos hábitos sociais e envelhecimento da população espera-se
um aumento na incidência de 1,5% ao ano. Em consequência, aumentará a procura e necessidade
destes serviços.
Foi neste sentido que o Centro Hospitalar de Trás-os-Montes e Alto Douro, E.P.E.
inaugurou em 2008 o primeiro Centro Oncológico fora dos grandes centros urbanos, apresentando-se
como uma mais-valia para a região, evitando as antigas deslocações dos pacientes para o centro da
cidade do Porto, com o objectivo de se traduzir numa diminuição de custos financeiros e sociais para
o estado, utentes e suas famílias.
Após estas considerações, surgem as seguintes questões: “A que nível será atingida a
eficiência num serviço de radioterapia? Será que existem vantagens na existência de serviços de
radioterapia descentralizados geograficamente? Será que o serviço é produtivo? Qual o custo social
de uma região perante a inexistência de um serviço de radioterapia? Será que os equipamentos são
suficientes para a população existente? Será que os recursos humanos se encontram em número
adequado para a população existente?”. Foi a partir destas e de outras interrogações que emergiu o
tema de investigação – “Sobre a eficiência de um serviço de radioterapia”. Definindo como
objectivo principal averiguar vantagens e desvantagens sobre a existência destes serviços em
determinada zona geográfica.
Recorreu-se à pesquisa bibliográfica e análise documental para a elaboração deste trabalho
de investigação, sendo este um estudo do tipo observacional descritivo. Procede-se à investigação
dos custos directos para a realização dos tratamentos (sessões de tratamento e transporte), não sendo
tido em conta os custos sociais inerentes.
Assim, os resultados obtidos apontam para diferenças significativas quando comparando o
custo do tratamento no CHTMAD e na antiga instituição de tratamento, o custo médio na instituição
actual é de aproximadamente 13.235,34€, na instituição de referenciação anterior, o custo seria de
13.450,17€. Esta diferença seria mais significativa se houvesse oportunidade de contabilizar os
custos sociais inerentes à deslocação destes doentes para a instituição de tratamento anterior (IPO
Porto). A eficiência poderia ser superior se o número de pacientes tratados na instituição fosse
superior.

Radiotherapy has application in 50 to 60% of cancer patients. With the change of social habits and an aging population is expected to increase incidence of 1,5% per year. As a result, the demand and the need for these services will increase. This was why the Centro Hospitalar de Trás-os-Montes e Alto Douro, EPE, in 2008 opened the first cancer center outside of a major urban centers, presenting itself as an asset for the region, avoiding the old movement of patients to the center of Oporto, in order to result in a reduction of financial costs and social consequences for the state, users and their families. After these considerations, the following questions arise: “At what level will be reached the efficiency in a radiotherapy service? Are there advantages in the availability of radiotherapy services decentralized geographically? Is the service productive? Is the equipment sufficient for the existing population? Are the human resources in adequate number for the existing population?”. It was from these and other questions that emerged from the research subject - "On the efficiency of a radiotherapy service”. The main objective was to ascertain the advantages and disadvantages of the existence of these services in a given geographical area. In order to do so, literature research and document analysis was done for which gave rise to a descriptive observational study. Then proceeded to the investigation of direct costs for both treatments (treatment sessions and transportation), not being taken into account the social costs. Thus, the results indicate a significant difference when comparing the costs of a treatment at CHTMAD and the previous institution of treatment. The average cost in the current institution is around 13.235,34€, at the oldest referral institution the cost would be 13.450,17€. This difference would be more meaningful if the opportunity arose to account for the social costs involved in moving these patients to the institution of treatment (IPO Oporto). The efficiency could be higher if the number of patients treated at the institution was higher too.
