Qual dos indicadores de composição corporal e antropométricos se relaciona melhor com a pressão arterial em diabéticos tipo 2

Objetivo: O objetivo deste trabalho foi identificar qual dos indicadores de composição corporal (percentagem de massa gorda) ou de antropometria (perímetro da cintura e o índice de massa corporal) melhor se relaciona com a pressão arterial (PA) em indivíduos com Diabetes Mellitus (DM) tipo 2. Métodos: A amostra foi constituída por um total de oitenta e oito indivíduos com DM tipo 2 e com hipertensão arterial (sexo: 44 mulheres, 44 homens; idade: 66,42 ± 6,10 anos) incluída no projeto “Diabetes em Movimento®”, um programa comunitário de exercício físico para indivíduos com DM Tipo 2. Foram medidos por impedância bioelétrica, a massa gorda (MG), a percentagem de massa gorda (%MG) e a massa isenta de gordura. Foram ainda avaliados o perímetro da cintura (PC), o peso e a estatura para determinação posterior do índice de massa corporal (IMC). Avaliou-se também a pressão arterial sistólica (PAS), a pressão arterial diastólica (PAD) e a pressão arterial média (PAM) com recurso a um esfigmomanómetro (M6 Comfort, Omron). Resultados: No sexo feminino, verificou-se que na correlação entre a PAS e o IMC (r= -0,083; p= 0,591), entre a PAS e o PC (r= 0,002; p= 0,991) e entre a PAS e a %MG (r= -0,138; p= 0,370) não foram encontradas diferenças estatisticamente significativas. Verificou-se um padrão idêntico em relação à correlação entre a PAD e o IMC (r= 0,182; p= 0,237), a PAD e o PC (r= 0,205; p= 0,182) e a PAD e a %MG (r= 0,183; p= 0,234). As correlações entre a PAM e o IMC (r= 0,053; p= 0,734), entre a PAM e o PC (r= 0,115; p= 0,458) e entre a PAM e a %MG (r= 0,021; p= 0,891) também não apresentaram diferenças estatisticamente significativas. No que diz respeito ao sexo masculino, as correlações entre a PAS e o IMC (r= 0,264; p= 0,083), o PC (r= -0,132; p= 0,394) e a %MG (r= -0,248; p= 0,105), e entre a PAD e o IMC (r= -0,198; p= 0,197), e o PC (r= 0,240; p= 0,116) e a %MG (r= -0,242; p= 0,113) também não apresentaram diferenças estatisticamente significativas. Por último, as correlações entre a PAM e o IMC (r= -0,267; p= 0,080), entre a PAM e o PC (r= -0,218; p= 0,155) e entre a PAM e a %MG (r= -0,284; p= 0,061) também não apresentaram diferenças. Foram ainda testadas as correlações entre as variáveis IMC, PC, %MG, MG, e massa isenta de gordura tanto no sexo feminino como no sexo masculino e a correlação é estatisticamente significativa. Conclusão: Relativamente à relação entre as variáveis antropométricas, o PC e o IMC, e a variável de CC, a %MG, com a PA, os resultados permitem concluir que estes indicadores não se correlacionaram de forma positiva com a PAS, PAD e PAM numa população com DM tipo 2 hipertensas. No entanto, estes indicadores têm todos uma muito alta correlação positiva entre si, pelo que uma alteração em algum deles, poderá corresponder a uma modificação diretamente proporcional nos restantes.
Introduction: The objective of this study was to investigate which of the measures of body composition (percentage of body fat assessed by bioelectrical impedance) or anthropometry (waist circumference and body mass index) is better related with arterial blood pressure in type 2 diabetic subjects. Methods: The final sample was formed by eighty-eight subjects with type 2 diabetes and high blood pressure (44 women, 44 men; 66,42 ± 6,10 years), within the project “Diabetes em Movimento®”, a community exercise program to people with type 2 diabetes. Using bioelectrical impedance it was possible to assess the body fat, the percentage of body fat and the fat-free mass. By the other hand, it were also assessed the waist circumference and the weight and height, in order to calculate the body mass index. We also assessed the systolic, diastolic and average blood pressure using a sphygmomanometer (M6 Comfort, Omron). Results: In women, analyzing the correlation between systolic blood pressure and body mass index (r= -0,083; p= 0,591), between systolic blood pressure and waist circumference (r= 0,002; p= 0,991) and between systolic blood pressure and percentage of body fat (r= -0,138; p= 0,370) we did not found any statistical significance. We could verify the same regarding the correlation between diastolic blood pressure and body mass index (r= 0,182; p= 0,237), between diastolic blood pressure and waist circumference (r= 0,205; p= 0,182) and between diastolic blood pressure and percentage of body fat (r= 0,183; p= 0,234). Finally, the correlations between average blood pressure and body mass index (r= 0,053; p= 0,734), between average blood pressure and waist circumference (r= 0,115; p= 0,458) and between average blood pressure and percentage of body fat (r= 0,021; p= 0,891) did not shown great statistical significance as well. The same happened in men regarding the correlation between systolic blood pressure and body mass index (r= 0,264; p= 0,083), waist circumference (r= -0,132; p= 0,394) and percentage of body fat (r= -0,248; p= 0,105), and between diastolic blood pressure and body mass index (r= -0,198; p= 0,197), waist circumference (r= 0,240; p= 0,116) and percentage of body fat (r= -0,242; p= 0,113). Finally, the correlations between the average blood pressure and body mass index (r= -0,267; p= 0,080), waist circumference (r= -0,218; p= 0,155) and percentage of body fat (r= -0,284; p= 0,061) did not shown any statistical significance. They were also tested correlations between body mass index, waist circumference, percentage of body fat, body fat and fat-free mass in female and in male, demonstrating a statistical significance correlation. Conclusion: Respecting the relation between the anthropometric (waist circumference and body mass index) and body composition (percentage of body fat assessed by bioelectrical impedance) variables and the arterial blood pressure we verified that none of this indicators was correlated in a positive way with systolic, diastolic or average blood pressure, in a type 2 diabetic and hypertensive population. In turn, all of these indicators have a very high positive correlation with each other. So, we can conclude that a change in any of them may correspond to a proportional change in the others.
