Efeitos de um programa combinado de intervenção psicomotora e treino cognitivo mediado por sistema computorizado na população idosa

Introdução: O envelhecimento é experienciado por um conjunto de alterações em vários níveis -biológico, psicossocial, neurológico, cognitivo e psicomotor- e está associado a uma grande probabilidade de incidência de doenças neurodegenerativas. A Psicomotricidade adequa-se às características da população idosa, de forma a evitar o deterioramento psicomotor associado ao envelhecimento, como também a manutenção dos fatores psicomotores. O estudo teve como objetivo principal observar o papel da intervenção psicomotora combinada com o treino cognitivo mediado por sistema computadorizado COGWEB® no perfil cognitivo e psicomotor, após 3 e 9 meses de intervenção individualizada. Materiais e Métodos: Concebeu-se um estudo longitudinal e quasi-experimental. Os participantes do estudo foram idosos residentes da Residência Sénior do Hospital Terra Quente. Os utentes cujo diagnóstico permitia foram avaliados quantitativamente pelo Exame Geronto-Psicomotor (EGP), o Mini-Mental State Examination (MMSE), e o Montreal Cognitive Assessment (MoCA), e os idosos com um comprometimento neurológico e psicomotor grave foram avaliados de forma qualitativa. A Escala de Rankin modificada (ERm) e o Questionário de Avaliação Clínica da Demência (Q-ACD) foram instrumentos utilizados para caracterizar a amostra no que diz respeito à capacidade funcional e às limitações cognitivas, respetivamente. Resultados: A amostra foi constituída por 19 participantes (x̅= 86,3; ± 6,15 anos de idade), 9 do sexo feminino (47,4 %) e 10 do sexo masculino (52,6 %). Destes, 2 eram analfabetos (10,5%), 3 tinham escolaridade superior à 4ª classe (15,8%) e 14 tinham a 4ª classe (73,7 %). 42,1 % dos idosos eram casados (n=8) e 57,9 % eram viúvos (n=11). Metade dos idosos (47,4 %; n=9) tinham deficiência moderadamente grave e 63,2% (n=12) tinham diagnóstico demencial. Inicialmente, em relação ao perfil psicomotor, 15 (93,8%) idosos apresentavam um desempenho psicomotor abaixo da média de referência para a faixa etária e 1 utente (6,3%) um perfil psicomotor acima da média, e no que diz respeito ao perfil cognitivo, 81,3% dos participantes tinham defeito cognitivo (DC) quando avaliados pelo MMSE e 100% dos alfabetizados apresentavam DC quando avaliados pelo MoCA. Após os 3 meses de intervenção verificaram-se melhorias significativas no perfil psicomotor (p= 0,002) e no perfil cognitivo, quando avaliado pelo MoCA (p= 0,033). No final de 9 meses de intervenção observaram-se melhorias significativas no perfil psicomotor (p=0,000). Qualitativamente observaram-se evidentes melhorias na praxia global, socialização e na expressão emocional/afetividade, diminuição da labilidade emocional e aumento da resposta sensorial. Conclusões: Os resultados do presente estudo apontaram para uma influência positiva da intervenção combinada em relação ao perfil psicomotor e cognitivo do grupo de idosos institucionalizados. É importante realizar mais estudos na área da Psicomotricidade, que permitam avaliar de forma rigorosa a eficácia de uma intervenção individualizada.
Background: Aging is experienced by a set of changes at various levels- biological, psychosocial, neurological, cognitive and psychomotor- and is associated with a high probability of incidence of neurodegenerative diseases. Psychomotricity is adapted to the characteristics of the elderly population, in order to avoid the psychomotor deterioration associated with aging, as well as the maintenance of psychomotor factors. The main objective of this study was to assess whether the role of psychomotor intervention combined with computer-mediated cognitive training COGWEB® improves the cognitive and psychomotor profile after 3 months of individualized intervention, and if the same programme promotes the same effect at psychomotor level after 9 months of intervention in the elderly population. Materials and Methods: This study longitudinal and quasi-experimental study was designed. The study participants were elderly residents of the Senior Residence of Hospital Terra Quente. The patients whose diagnosis was possible were quantitatively assessed by the Geronto-Psychomotor Examination (EGP), the Mini-Mental State Examination (MMSE), and the Montreal Cognitive Assessment (MoCA), and the elderly with severe neurological and psychomotor impairment were assessed qualitatively. The modified Rankin Scale (mRS) and the Dementia Clinical Assessment Questionnaire (DCA) were instruments used to characterize the sample in terms of functional capacity and cognitive limitations, respectively. Results: The sample consisted of 19 participants (x ̅= 86, 3; ± 6,15 years old), 9 females (47,4 %) and 10 males (52,6 %). Of these, 2 were illiterate (10,5%), 14 were in the 4th grade (73,7%), and 3 had an education higher than the 4th grade (15,8%). 42,1% of the elderly are married (n=8) and 57,9% are widowed (n=11). Half of the elderly (47,4%; n=9) have a moderately severe disability and 63,2% (n=12) have a dementia diagnosis. Initially, regarding the psychomotor profile, 15 (93,8%) elderly had a psychomotor performance below the reference average for the age group and 1 user (6,3%) had a psychomotor profile above the average, and regarding the cognitive profile, 81,3% of the participants had cognitive defect (CD) when assessed by the MMSE and 100% of the literate had cognitive defect when assessed by the MoCA. After the short-term intervention, there were significant improvements in the psychomotor profile (p= 0,002) and in the cognitive profile assessed by the MoCA (p= 0,033), and in the long term, there were improvements at the psychomotor level (p=0,000). Qualitatively, there were clear improvements in overall practice, socialization, emotional expression/affectivity, decreased emotional ability, and increased sensory response. Conclusions: The results of this study point to a positive influence of the combined intervention in relation to the psychomotor and cognitive profile of the institutionalized elderly group. It is important to carry out more studies in the area of Psychomotricity, which allow a rigorous assessment of the effectiveness of an individualized intervention.
