Gestão da qualidade de vida e da dependência em idosos institucionalizados nas organizações do terceiro setor

As condições crônicas e por vezes incapacitantes – processo pelo qual uma determinada
condição (aguda ou crônica) afeta a funcionalidade dos idosos - tendem a se manifestar com o
avançar da idade. Sabe-se que normalmente tais limitações não são fatais, não obstante tende a
comprometer de modo significativo a qualidade de vida da pessoa idosa ao afetar o desempenho da
funcionalidade e, em consequência as atividades de vida cotidiana. Em todo o mundo moderno o
aumento exponencial na quantidade de idosos é uma realidade que aponta para a melhoria dos
indicadores de saúde, cresce também a demanda por serviços de saúde voltados para a população
idosa e a necessidade de utilização de instrumentos de avaliação funcional. O Índex de
Independência nas Atividades de Vida Diária (AVD), desenvolvido por Sidney Katz, é um dos
instrumentos mais antigos. Este estudo teve por objetivo conhecer a Qualidade de Vida (QDV) das
pessoas idosas institucionalizadas, sua relação com as variáveis descritivas, sócio-econômicas e a
importância da gestão institucional neste contexto. Optou-se por um estudo exploratório-descritivo
e transversal no qual foi utilizado como instrumento de recolha de dados, um questionário biorelacional,
uma grelha de avaliação da Qualidade de vida, Índice de Katz modificado e um
questionário aos gestores da instituição.
Participaram no estudo 130 utentes de sete instituições para idosos do concelho de Oeiras.
Constatou-se que os fatores que interferem na QDV variam em função do gênero 81,5% sexo
feminino e 18,5% sexo masculino do total da amostra, da existência de dor (p= 0,014) e existindo
uma correlação positiva entre o índice de Katz e à qualidade de vida (p< 0,001). Este resultado
significa que quanto maior a independência do idoso maior é a sua qualidade de vida. Na escala do
índice da QDV 56,2% dos idosos do total da amostra se encontram integrados no grupo sem
qualidade de vida. Os idosos do gênero masculino apresentaram valores do índice de QDV
superiores aos apresentados pelos idosos do gênero feminino.
Por outro lado constatamos que o índice de qualidade de vida não difere em função do
grupo etário, estado civil, nível de instrução, existência de ambientes adaptados, existência de
fiscalização dos serviços por parte dos gestores, promoção da autonomia e adoção de medidas para
prevenção de quedas da pessoa idosa institucionalizada.
Chronic and sometimes disabling conditions - process by which a certain condition (acute or chronic) affects the functionality of elderly people - tend to manifest with advancing age. It is known that such limitations are not usually fatal, although it tends to significantly impair the quality of life of the elderly by affecting the functionality and performance, and as a consequence the activities of everyday life. Throughout the modern world the exponential increase in the number of older people is pointing to the improvement of health indicators, growing also the demand for health services aimed at the elderly population and the need for use of functional assessment instruments. The Index of Independence in Activities of Daily Living (ADL), developed by Sidney Katz is one of the oldest instruments. This study aimed to know the quality of life (QOL) of the institutionalized elderly, their relationship with the descriptive variables, socio-economic and importance of institutional management in this context. It was chosen an exploratory-descriptive and transversal study in which were used as instruments of data collection, a bio-relational questionnaire, an evaluation grid of the quality of life, a modified Katz Index and a questionnaire to managers of the institution. One hundred and thirty users from seven nursing homes in the municipality of Oeiras participated in the study. It was found that the factors that affect QOL vary by gender 81.5% female and 18.5% of the total male sample, by the existence of pain (p = 0.014), and there is a positive correlation between the Katz Index and the quality of life (p 0.001). This result means that the greater independence of the elderly the higher quality of life. In the scale of the QOL index, 56.2% of the elderly of the total sample are integrated in the group without quality of life. The elderly male presented QOL index values greater than those presented by the elderly female. On the other hand, we see that the quality of life index does not differ according to age group, marital status, education level, availability of suitable environments, existence of supervision services held by managers, promotion of independence, or the adoption of falls prevention measures for institutionalized elderly.
